Reprints of a single article may be purchased through the Department of Publications at Joint Commission Resources. To order reprints, please use the form below.

Note: The minimum order for reprints is 100 copies. For smaller orders or to obtain a single copy of an article, please refer to our Permissions information or contact the Permissions Editor at 630/792-5454.

Please print out the reprint form (to download a PDF of the form click here), complete and mail with payment to the address shown below. You will be contacted within five to ten business days with a response to your request. When finished with the reprint form, please send it along with a check for the correct amount (payable to Joint Commission Resources) to:
